Critique: Asking for overall critique, note the model is not a professional she's just a good friend.

Goal of the image was to having heavy shadows and capturing her eye as sharp as I possibly can, as well showing the color in her eyes.

I think you got the shadows you were after.

Did you use a softbox or just an off camera flash?

off camera medium softbox studio strob light - right of the frame
