Default Fussy baby shoot

Attempted to do a makeshift shoot last week, however with the parents showing up late, loosing my natural light and the baby being fussy (and gassy) I was unable to get too may good shots. Here is one that I think is passable, but I could use some critique on what to change/add to the image. I did photoshop some light from the right side to make up for the lake of natural light.

Focal Length 50 mm
ISO Speed 100
Exposure Bias 0 EV
Flash Off, Did not fire

The frame is good, I like it. And the face looks relatively sharp. I got to say, my eye keeps drifting to the blown out area in the upper right corner. I also noticed that the babys arm and knee seem to be blown out little, especially the arm--its lost all of the soft baby-skin apperance.

One last thing. The debris on the lower left (looks like crumbs or something) could be removed to clean up the image.

there's something on the wood floor, on the left of the image. i would get that out of there too.
