To me, the most distracting thing is the tight crop on the bottom. To me it's never a good idea to crop off the chin. Top of the head is ok, chin not. Plus with his hand in the mouth I would have wanted to see more of the hand.
Even though I know (and agree with) you wanted to focus in on the emotion with your tight crop, it's the crop itself that is most distracting to me. |

I really like the emotion here, but I agree with others on the crop. I can't tell if that's his hand in his mouth or someone is punching him in the mouth...
