Very good idea on the pose there are just 2 things tht jump right out at me.
While she is looking down, she is only looking down with her eyes and more pronounced head tilt would better convey the "looking down" as it is it kinda looks like she blinked. Seeing both of there eyes would help a lot. The other thing that is mildly distracting is that I can see her bra under her shirt. As I work my way down the image I get stuck there. I little post processing would take care of that. The idea behind it is great though, keep it up!

I was having trouble with what was bothering me too. I really like the idea of the shot a lot, but it is your depth of field. Their faces are in great focus, but the heart, belly, and hands are in soft focus at best. I didn't see it until I look at it in the larger format, then it jumped out at me. Try shooting it with an f/8 or f/11.
On small compositional notes, the gap between their arms on the right is slightly distracting. It looks like he is squeezing her belly, not cradling it 'happening' to make a heart..it should look more natural or accidental. I would like to see a little more head room too, it looks like their heads are cocked because the frame is too low, you need enough to make it look like he could stand up without cutting his head off. Think eyes on the third line. His jeans need to be darker too, they look too much like a skintone. I really, really like the shot and think the couple will treasure it.

stockphotojourney, thanks! Those are two great suggestions! I did notice the bra, but my photoshop trial just ran out, so I'm going to have to touch that up, once I buy the program. I will keep that head tilt in mind, next time!

In addition to the above comments, may you could photoshop the bracelet and watch out. For some reason my eyes keep being drawn to them. I also think it would be nice if everything was not so dark. Maybe a lighter background. Overall, it is a lovely photo. Congrats on your pregnancy. What a lovely what to remember it.
