Soft light & bleached effect - how is it achieved

I was wondering if anyone knew how this sort of photo is achieved - the soft bleachy look?

ok, well....

1) its been fairly heavily photoshopped.

2) it looks like a nice large reflector has been used.

if you look at what should be in focus, and what actually is, you can see that there has been a lot of softenning/blur in PP
