Strap gets wrapped around my wrist twice, camera in hand. My strap almost NEVER goes around my neck.

I do the same thing when I'm shooting. The camera goes around my neck only when I'm moving around and I want it closer than my bag.

As for harnesses,
You can check for the Blackrapid product.
BlackRapid.com

Lowepro is also offering some products.

I often don't use any strap at all. When I am using a strap it's a Blackrapid R-Strap.

I always use a strap, but not around the neck, I wear it cross-body (strap on right shoulder, camera at left side/back). I use a regular neoprene strap, though, not a blackrapid r-strap, because I frequently put the camera on a tripod and always have a quick release plate on the body. I've suffered from RSI for over a decade and have very little grip strength and my hands can't bear a lot of weight, so I never take the strap off--it's my 'safety net' for catching the camera if I accidentally drop it. And I do. (sigh).

For those of you wrapping the strap around your hand--is there a reason not to use a handstrap instead?

Most are made for bodies without a vertical grip, and even those that are (say for a D3 or D2 series) are only useful if the camera is horizontal (they dont work for using the vertical grip, which is the entire point).

I switch from the neck to the hand depending if i'm shooting / moving around / break.
The camera goes often in the bag turned on as well but sometime I want it closer.
