Default Macro Flash

I got sick of the 3mm wide DOF I was getting with my Macro lens and busted out my flash for some smaller apertures. I tried a few different things to get the results I wanted:

Directly on hot shoe:
Worked fine with omni bounce on flash while I was over a foot away, but my lens started getting in the way as I got closer.

Placing flash on ground next to subject:
Did fine, didnt have the accuracy I wanted and stray leaves casted huge shadows.

Holding in left hand:
Ideal for lighting, not so much for focusing and holding the camera steady.

I am aware that Nikon offers a macro flash system that clips to the lens filter, but I would like to use my SB-600 to save some cash. Does anyone use a bracket that allows one or two flash units to be used effectively with macro?
