Default what happened?

I had three shots on my card that look like double exposures. I shoot jpeg and raw and both picturews are the same.

Any idea of what could cause this?

I know the D90 has a setting to allow multiple exposures. Perhaps your camera was accidentally put into this mode. But look on the bright side - at least you weren't shooting the neighbors toddlers right after a haunted house.

It's a D90. I will have to check and see how it does multilple exposures. Previously when I wanted to shoot multiple all I had to do was keep my finger on the button. I didn't know it would double expose.

If you come to the conclusion that it's not any camera settings causing it, then I would say that it's the memory card. Are you using an off brand or a cheapy?
