Default 2 layers?

So the objects in front are from a second picture, right? Like second layer?

No - it is a side effect of how cameras work. Go back and read the first post, which gives an illustrated example of how to get this effect.

Are the two lights in the front not being affected by the bokeh because the other lights are in further away?

The two lights in the front are in focus. Bokeh refers to what happens to points of light as they get further away from the zone which is in focus.
