I always shoot in full manual. If I'm indoors without a flash I'll generally be at 400 or 800 on my K10D. There is very little noticeable grain below 1600, and even 1600 is fully useable (and barely indistinguishable from lower ISOs once I run it through NoiseNinja).

Outdoors in good light I will of course shoot at ISO 100.

Manual, I find if the other settings are coming in to low (i.e. on Program mode) I'll bump up the ISO until I can get a shot with the equipment I have/don't have (tripod mainly).

I generally shoot program mode so I can shift shutter speed and aperture at will, and use spot metering, manual focus, and always manual ISO. I shoot at 100 (my camera's lowest), and only go up from there if there's a need or if I'm experimenting.

Because of shooting film until recently, I'm usually comfortable about raising to 200, and wary of shooting at 400. But raising the ISO at all is a pretty rare occasion for me.

It's manual for me.
I have been known to be lazy and leave it on auto (i.e. shooting my husbands hockey games).

I'm pretty much manual ISO 90% of the time. The only time I'll switch to Auto is if I'm shooting under low-light conditions and am already open as wide as my lens allows and shooting as slow as I'm comfortable with.
