Default grainy/blurry photos

need a little help.. i have a nikon d90 and these were shot with a 50mm iso 200, this image was 1/200 of a second f13. i am using the single point focus and my sb600 my images just are not coming out consistently clear and i am just confused as to why i have never had this problem before (always used auto, auto focus never selected the focus point) i am focusing on the eyes. any ideas?
the first image represents the settings listed above, the other images are various settings but same problem.

I am not sure what help can be provided here. I can't see too much wrong with the images, though they might be a little soft. It's difficult for me to tell. Are these shots straight out of the camera or have they been resized?

If you find the images soft, then given the aperture you are using and the lens focal distance it is unlikely to be due to missed focus. Therefore a list of possibilities that I can think of, in no particular order, are:
1. Your in camera JPG setting may be set to something different from before, i.e. lower quality jpg setting.
2. Your sensor may be slightly misaligned now
3. There may be an issue with your lens

I am not sure whether any of this is accurate or helpful, but thought you might appreciate a response!

I don't know if something changed between your previous shoots and this one (RAW instead of JPEG, new lens, bumped a program setting), but pulling that middle image into GIMP and applying a bit of a high-pass sharpening gave it some pop.

Not sure if/how you're processing these images, but a bit of sharpening might give them the edge you're looking for.

thanks for trying! i see the issue when Zoomed to 100% in ps.. these shots are straight out of the camera i shoot raw +jpeg fine and i double checked my camera and this is still my setting on my camera, as far as the sensor being misaligned, or the issue with the lens, i don't seem to have this problem when shooting on auto auto focus, i am going to do some test shots with different lens tomorrow, i pray its not my lens.
