400D with lense attached, telephoto lense, lense hoods, filters, lense cleaning kit, shutter release cable (when I remember to pull if out of my large camera bag), model release forms, pens, extra canon battery, extra memory cards, small notepad, pocket calendar and printed PDF of "the Photographers Right"

Let's see:

Canon EOS 350D
Sigma 70-300mm AF lens
Sigma 17-55mm AF Lens
cable release
extra compact flash cards
extra battery
star filter
2 circular polarizers
Handheld GPS receiver, (for not getting lost)
In winter, no less than five chemical hot packs (one for each glove and three to keep my batteries warm and functioning)
reading glasses
pen
Canon manual
business cards
notebook
maps, as needed
Oakley Razor sunglasses and/or another set of sunglasses with persimmon lenses

That's about it...

350D with 28-135mm 3.5-5.6 IS USM
18-55mm 3.5-5.6
75-300mm 4-5.6
50mm 1.8
430EX Flash
And then I switch between my Voigtlander and my OM-1 depending on how I feel. Both shoot film, and currently are shooting slides.

Other than straight up gear:
I ALWAYS bring with me a printed version of Bert P. Krages "Photographer's Right" pamphlet, to educate people about street photography and the rights of photographers: http://www.krages.com/phoright.htm

I also bring some small manuals for the 350D just incase,
some lens cleaner,
and LOTS of film. Like 20 rolls.
