First off, it is a shame to mess with perfection. I love the attitude in this shot. But since you asked:

In Photoshop CS2. I layered via copy, applied an under-painting filter with canvas texture and changed the blend mode to lighten. I copied that layer, added a rough pastel filter with canvas texture and blended in overlay mode. Copying that layer, I added a median noise filter and blended in lighten again. I used the history brush to bring back those amazing eyes, and finally, I tweaked the whole thing with shadows and highlights.

I actually had quite a bit more fun with this one than the last WWYD.

I started off by using the pen tool to outline the young lady and then I turned that into a selection. Next, I found a picture on the web which would be suitable for my concept. Again, I duplicated the base layer then selected the billboard and erased the contents. Next I created another layer which I dragged below the duplicate layer then used brightness and contrast to make it completely white. I then created a fill layer for the billboard background which I placed above the white layer then made it a clipping path with the white layer. With the duplicate layer selected and the billboard itself selected, I painted in the colour from the fill layer. Then I went back to the original and dragged the little girl over to the other image and then transformed it to match with the perspective of the billboard. Finally I added the text and did the same transformations to alter it's perspective, too.

Got Cookies

I couldn't seem to find a way to make the lettering look smoother... I don't know... maybe it was the font I chose. Any advice in this area would be appreciated.

Note after posting:
After seeing it posted here, the letter doesn't look as bad as I thought. Still any suggestions on improving the look of the lettering would be appreciated.

The lettering looks fine to me - I like your take on this, it is very good!

I started out intending to do a monotone but when doing the hue sat I ended up not ticking the colorize button and ended up with some strange effects

I then coloured the eyes using a colour blending mode with a hue/sat layer attached to get (to me) perfect colour and saturation

Then not happy with that decided to erase the layer above the background to bring back the natural hair colour (best I could with messy hair )
