Default Head Shot Quote

One of my clients told her HR dept about me b/c they are looking to get head shots for the entire management and sales staff (45 people).

There is no guarantee that I will get the job, I am competing with others. My question is HOW do I begin to write a proposal for this? I know I have to come up with a competitive price but what else do I need to include. This is what I have come up with so far:

1. Make-up date for head shots (absentees and such)
2. When they should expect proofs
3. How long the session will be
4. Backdrop color (any suggestions)
5. An explanation of HOW they can use the pics
6. A company description (they sell to the gov't, so I must mention minority ownership )
7. Cost as new employees come on board...

I know that they will require a digital version b/c they use the photos in their proposals.

Anybody have a sample propal they want to share with me...
