Default Image Reprint Release Form?

Does anyone have an "image reprint release form" that they use to give out with high-res CDs should a customer buy one?

In "legalize" terms I believe it is actually called a Creative Commons Aggreement, and you can find a copy of it here: Creative Commons Legal Code

It's long, but you can alter the agreement to suit your needs/add your biz name to it somewhere, etc.
I have this altered for my personal taste and I have to say I changed it quite a bit, and only allow printing rights and such, I do not allow altering the works, etc. , (of course you and I both know control over alot of this is a mute point anyway.)

I have my file saved as a PDF

When I have a customer meet the dollar amount I have set for print orders, they have the option of a high-res disc, which I then embedd this PDF file directly onto the disc.

Maybe something like this will work for what you need

You don;t need the full agreement. As long as there's a clause in the contract that describes usage rights, then you're covered.

I have a very simple one that I give to clients if you want a copy.
