Question Permission Release Form?

Hi all,
I recently took a photo for my aunt who is having a book published. (She needed a head-shot for the back cover.)
Anyway, she is sending all the final stuff to the publisher this week and today they sent her an email with all of the size specifications of the photo (that's all taken care of -- I think) and the request for a permission release form. This is what the publisher said:
"All in-text images must be free and clear of all rights. Please have the photographer or artist of each in-text image sign and send (via e-mail or postal mail) the attached Permission Release form. If they are all by one artist one release will cover those. If by different artist, each artist must send in a signed release. We must receive your permission releases with the final draft of the manuscript. We will proceed with the production of your book and remove any graphics for which you do not provide permission releases."
And well, they didn't attach a form! So my aunt is in a tizzy with deadlines and needs me to get a form to her ASAP. Can anyone give me an example of what such a form would look like?

Wouldn't it be simpler to reply to the publishers that the form is missing, and have them send it through?

A location in your profile would be helpful.

I was wondering what country's laws may be involved, but directly contacting the publisher seems a straight forward solution.
