Do you use order forms or receipts?

Just wondering if anyone makes their own order forms or receipts, or if they get the templates from somewhere (or if you don't use them at all?). I take most of my print orders online (smugmug), but I'd like to start going through WHCC for proof books, albums, etc. but want to make it easy for my customers to order these. Any suggestions?

Photo Cart is great for online ordering followed by fulfillment wherever you wish.

For on-site orders at event's I design my own forms. There are also places that will design them for you, let you use your design, or pick from a pre-maid form.

Google No carbon Required order forms or some such for ideas.

i created my own.. excel is the way to go for ease and "cheap" ...
since i am doing my own fulfillment right now thats what I want to use and maybe later when I upgrade or start making alot of money, etc than I will go with Photocart as it does look great

I make all my own templets in Microsoft Entourage.
