Release Forms

Anyone know where I can find templates for release forms? I can find model release forms but not release forms for selling a disc of images. I want to include a limited rights release which allows them to reproduce the photos so they can go to a local shop to print if needed. Just can't seem to find anything like that online.

Most model releases have a section similar to this that shouldnt be too hard to isolate. I know the template I found was divided into sections, one of which was a rights release/grant.
