Default Release forms/Licenses ???

Let me start off by letting everyone know I shoot Motorsports, so here's my question since I've never delt with this type of photography....

I've been asked to do a Promo shoot for a local "metal" band that is gaining alot of attention. I have researched myself into confusion lol. What type of release forms or licenses do I need them to sign ?

I'm wanting to do a shoot, give them web versions to use for promo reasons.

But one member stated they needed these shots for flyers and maybe C.D cover also. So how in the world do I handle this ? A C.D with hi-res images at a given price with a certain license agreement ? Maybe the Attribution licenses?

I'm stuck haha, just hit me with some info please !

Limited Rights Usage copyright release
Model Releases (for each member)

The first grants them limited usage rights, which means that they can use the images for a given period of time (that you set) and for particular uses (that you list).

The second is a release that allows you free use of the images.

Ok..Thanks for the replies.

From the info I've gained I hope I'm in the right direction now.

Have them each sign a model release
Do the shoot (at set price)
Then offer a "limited rights" license (at set price)
They recieve a C.D of full res images to use however they want for a set amount of time.

* So for the photographer to gain credit for the published images, for example ,they sell 8x10 band prints at the show. They would have to have my name on the print ...right?

Or is this something that needs to be agreed to in the license ?

Hopefully I'm on track here haha.
