What documents needed to sell all rights to a photograph?

I intend on selling a picture to a business for their unconditional use - flyers, brochures, billboards,etc...

What should I provide them other than the disc ? - Is there a standard form that would protect their purchase, other then my word that I won't use it anywhere else??

Sounds like you need a contract. There's no standard one as they need to be drawn up to suit your needs but there are some templates online that you can work from. For extra security you might want to get it checked by a legal representative.

From your post it sounds like you're signing over all rights - you can give a company full use of your images and still sell to another - unless they are asking for full exclusivity in which case you should be charging big monies
