Your straight out of camera image doesn't have much skin texture to begin with so very little smoothing is needed if any at all. I would focus more on removing blemishes, acne, etc. Perform a slight skin smooth if needed, then do a local sharpen as the last step. Sharpening the eyes, lips, whatever is in focus. Sharpening the eyes might require different settings than sharpening the lips or hair would.
global sharpening should always be the last step done in my opinion. I use different sharpening settings for web, resolution size, print, etc. Sharpening an image for web might look good on a monitor but it might look under sharpened when printed.

A nice trick is to hold down the alt key while moving the sharpening sliders to better see what you are doing.
This was the best I could do with just basic LR settings. A big part of the issue is it's not terribly sharp to start with and trying to make it tack sharp @ 100% in LR is never going to work....It won't even work in PS but could probably do a little better.
