Too much flash. The amount of light especially coming from the area of the lens shows too much of the baby's facial blurbs. If possible, it's best to use existing light. If your camera has a setting to allow you to adjust the amount of flash, use it. Use some tissue in front of the flash to diffuse it. Also it's a bit yellow. Check your white balance. When in doubt, use your cameras manual WB setting if available.
A more direct shot would show off more of this gorgeous little babe. I hope you don't mind but I couldn't help myself. Will delete if requested.

Gaussian blur the skin. There are various ways to do it but I prefer to erase everything but the skin. Duplicate the layer, then select the layer you started off with. Add a fill layer and select a color. The baby has the white cloth at the bottom I needed to do some detailed erasing around so I chose black.
You should now have the original background, a black layer and a copy of the background. Select the copy and start erasing. Use a large brush for large areas. When you get to the detailed work you will need varying brushes depending on the detail. You always want a feathered brush, no harsh edges. Adjust the size for the size area you want to retain detail. For softer facial lines, bump the opacity of the brush down about half. I don't like to retain full definition here and I don't want to blur it out completely either. Once you're happy with what you have, hide the fill layer and make sure the background copy(face) is selected. Go to the Gaussian blur and run the slider down, and then slowly up until you're happy with the complexion.
