Permission Form

This is the first time that I have posted anything in this forum and I hope that I am posting in the correct area. I was searching on google for permission forms to print & I came across a bunch of different ones. Some of them were plain & to the point & then others were more complex & harder to read. Does anyone have any recomendations on which one I should use & what's the best? Could you attach what you use or give me a link? This would help me a lot! Thanks in advance!!! :)

Thank you for the copy of a model release form! It's something definitely worth keeping in my camera bag - now if I could only work up the nerve to actually speak to strangers I'd be set :p

How are you at assuming a role? Remember old movie reporters with the fedora and the card stuck in the band? Wear a hat with "Photographer" printed on a card stuck on it somewhere, then watch people come to you. Forget who you are for a while.

I hear ya - i'm getting a little better at it, but it's still nerve racking for me!!.. :cool: np on the form :)
