Well the picture was definately taken an 1/5 sec. The EXIF doesn't lie and the fan is blured like it was shot at that speed. Either you aren't setting the shutter speed correctly or the camera is somehow overiding your settings to expose as it sees fit (I would doubt this though).
Ignore the flash in this context as shutter speed will not perform as expected when using a flash.

shutter speed is working in manual mode but i think its not workin in Tv mode.. why????/
