I often forget to check the ISO as I don't use to do much with it, but when I do set the ISO higher in low light situations I usually forget to set it back, and I forget to check it the next time I use the camera.
Once I managed to take a lot of pictures with an iso of 1600 in full daylight and I didn't notice before I checked out the pictures on the computer and saw all the noise...!
