There are bunches of tutorials as well as an entire Flickr group. Basically, you create a gradient mask and apply some form of blur, and then adjust hue/saturation to make things a little more "toy-like." Here's a link to a photoshop tutorial.

Or, if you're really lazy, you could just use this.

One of the keys to getting the effect is to shoot from overhead, or in some way that the gradient direction you choose will simulate DoF in the shot.

If you want to go to extremes, you can create depth maps.

I'd also recommend taking a look at the NY Times feature on Vincent Laforet's tilt-shift photography.
