Very difficult...I guess I'd have to say backpack: Lowe Pro Mini Trekker when I want to take gear. Often I just take the camera and the 18-200. For the "in-between times" where I might want another lens/flash/accessories along I'll throw the "extras" in my Tamrac Velocity7 and carry the camera out.
I've used several "shoulder bags" (lowe, tamrac, calumet) but they very quickly get bulky and awkward....for that much gear I'd rather take the backpack. If I think I might need "quick access" I'll carry the camera on it's strap lens uncapped (normal setup)
If it's "take all my gear along"...I take the car.
