How is this done??

Can anyone give me some insight as to how these perfect looking clouds or smoke are created? Are they made in photoshop or are they actually taken from the sky and edited in. They are so well done in my opinion. Any insight would be awesome. Thank you

The plane is one image, the girl and the backgrund are another, and the cloud is a third. It's a great photoshop job.

Yup, she was spitting the plane out of her mouth, and they added the cloud to make it "different"
