You have an excellent photo, I like it. However I see little things that I would do different, but keep in mind that I did say you have an Excellent photo. 1st I would whiten her teeth just a little. The rest is just me as I see the photo. I like the original color of her hat, it adds more contrast. I would remove the lens blur; you have a very pleasing background. Just my thought, all for a penny.

I prefer the sharpness of the original. If you want to simulate depth-of-field that use lens blur rather than Gaussian. Moreover, keep in mind that if the eyes are in focus, then anything at the same depth plane must also be in focus, so the shoulder and hat would probably be fine, but could fade away as you leave the center. This will add a lot to the believability of the image that is missing now.
Also, since you have the patient open and on the table, might as well remove the red marks from under the eyes, chin, and brighten the eyes a bit by adding the missing "moon" to the lower left of the iris.
